Appeals procedure:  Due to recent insurance processing changes, certain insurance companies have changed their

processing of claims making it necessary for the patient (instead of my billing company) to appeal a claim on your

behalf. In that event, however, my billing company will be happy to assist you in your appeal. Feel free to also contact

me directly with any questions or concerns you may have.


Attention Blue Cross Blue Shield patients:  Please note that you may receive a check from Blue Cross Blue Shield

made payable to you which is actually intended to pay for my services. If that happens, please cash it and send a
